We have two Doctors and two Nurse Practitioners here to meet your individual needs and provide medical treatment to the entire family through all stages of life. We also collaborate with other health professionals and utilize consultation or referrals as needed.

We know there is no such thing as one-size-fits-all cure, so we never use a one-size-fits-all approach to your diagnosis. AFHP provides patient advocacy and cost-effective care. We view our patients at partners in health care.
We offer a variety of preventative care services such as vaccinations, immunizations, and wellness exams.
We offer diagnostics for many acute symptoms such as ear infections, strep throat, flu, colds, and other illnesses.
You can rely on us to treat all of your chronic care needs such as hypertension, diabetes, elevated cholesterol, and others.
With most insurance providers this is not a covered benefit. The cost of this treatment is covered by the patient.
Annual: Please check with your insurance company to see if your plan covers physicals.
Motor vehicle accidents: We treat patients with injuries related to auto accidents. To avoid payment discrepancies we do not file claims to parties health insurance, full cost of visit is due at the time of service.
Work Related Injuries: We do not accept ANY work-related injuries. Referrals to providers who accept work- related injuries will be provided for further care.
Please check with your insurance company to see if your plan covers these procedures, and if applied to deductibles. These procedures typically are considered cosmetic procedures and not medically necessary.
Effective March 1, 2022, patient's with no insurance, self-pay patients will see an increase in cost for services rendered.
Effective April 1, 2022. The office will no longer be accepting new patients who are insured by a commercial plan. Our practice will remain open for NEW MEDICARE ADVANTAGE patients.
